Dear Dr Taha Aldeen
Unable to find an email address, I am using this arrangement to send an apology. I had a follow up appointment to see you on 11th November 2021 at 16.00 which got cancelled. Totally through my own error, the rebooking wasn’t noticed and diarised, so I missed it (Queens 19th Nov 14.00) unintentionally. I realise it must be frustrating for you, and just wanted to say sorry. Everything seems fine, following your treatment, so, being discharged is okay.
